ABSTRACT

The osteosarcoma (OSA) is the most diagnosed primary bone cancer in canine patients. This work reports a case of a canine, six years old, mongrel, female, intact, with an OSA in the hard palate. Physical examination detected a firm mass in the palate. Thoracic radiographs, hematological and biochemical exams, histopathological exams and computed tomography were requested. A chondroblastic OSA was diagnosed and the tumor was characterized by immunohistochemistry. There was never evidence of metastasis in this case. The treatment consisted of the combination of conventional chemotherapy, metronomic chemotherapy, and palliative care, aiming at greater survival and well-being of the patient since surgical excision was not possible due to the location and extension of the tumor. Osteogenic sarcomas of the hard palate are rarely seen and described in the literature. In this article we present a characterization of the osteosarcoma with uncommon localization in the hard palate.(AU)

ABSTRACT

Necrotizing sialometaplasia (NS) is a benign, self-limiting inflammatory entity that mainly affects the minor salivary glands located in the hard palate. Classically, NS is characterized as a nodule that evolves to a central ulcer. The most widely recognized triggering factor is an ischemic event. The diagnosis becomes a challenge in non-ulcerated NS cases which is essential to rule out the possibility of salivary gland tumors, especially the malignant ones. Here, we presented a case of a 32-year-old male patient with a 1-month complaint of a painful, slightly elevated erythematous area on the hard palate. Incisional biopsy was performed, and NS was diagnosed based on histopathological and immunohistochemical analyses. Clinicians should be aware of and consider NS as a differential diagnosis of minor salivary gland tumors, particularly when it presents as a non-ulcerated clinical aspect.

ABSTRACT

During development, bony changes in the palate are reflected in the palatal rugae. Therefore, we hypothesized that the palatal dimensions (PD) influence the shape and number of palatal rugae (PR). The objectives were to record the palatal rugae characteristics (PRC) and palatal dimensions (intercanine distance (ICD), intermolar distance (IMD), palatal height (PH) and palatal area (PA) in Classes I, II and III malocclusion patients and investigate their interrelationship, and statistically examine the possibility of predicting PRC with the PD. Four hundred eighty-one pre-orthodontic study casts of healthy patients with normal palate anatomy were grouped as Classes I, II and III and scanned using 3D cast scanner. The PRC, ICD, IMD, PH, and PA were recorded digitally using 3D enabled software. The data was statistically analyzed. A strong statistically significant difference was observed between PA and number of straight and wavy rugae. ICD and the number of straight rugae were also related. A weak correlation exists between malocclusion classes and PA. The remaining rugae characteristics did not exhibit any relation with palatal dimensions. PA is positively related to the number of straight rugae and negatively related to the number of wavy rugae. Bigger palates have more straight rugae and less number of wavy rugae. A weak correlation between PA and Angle's class I malocclusion exists. We also propose that PA has a developmental association with the number and shape of PR.

ABSTRACT

(AU)

Objective: To report an excisional biopsy of a peripheral osteoma in the right hard palate in a non-syndromic young female patient. Case Report: A 32-year-old female patient, black, and normosystemic sought assistance in the stomatological diagnosis outpatient clinic of the School of Dentistry of the Federal University of Uberlândia, Brazil, complaining about the appearance of a lesion in the palate region. After clinical and imaging assessment, it was decided to perform an excisional biopsy under local anesthesia of the lesion. Three diagnostic hypotheses were raised - peripheral osteoma, exostosis, or osteosarcoma. The material collected was sent for histopathological analysis, which confirmed the hypothesis of peripheral osteoma. Final considerations: Peripheral osteomas in the hard palate region are very rare. This lesion is usually asymptomatic, except when there is a slow-growing chewing impact during mastication, which may develop in cortical or spinal bone. Surgical removal is the treatment of choice and the rate of recurrence is rare. The case emphasizes the rare condition, considering only six similar cases were found in the literature. (AU)

ABSTRACT

Oral focal mucinosis (OFM) is an uncommon, asymptomatic, submucosal, slow-growing nodule representing a counterpart of the cutaneous focal mucinosis (CFM). OFM has a female predilection with the highest prevalence in the fifth decade of life. About 68% of OFMs occur in the gingiva and 14% in the palate. We present the case of a 41-year-old woman presenting a progressively growing mass on the palate, since the last 8 months. The diagnostic workup led to the diagnosis of an unusual OFM with the clinical presentation involving the gingiva and hard palate. This case report discusses the clinical and histopathological differential diagnosis.

ABSTRACT

The anterior palatal fistula is the abnormal communication between the oral cavity and nose that occurs after surgical closure of cleft palate, this occurs more frequently in hard palate defects. Tongue flap is indicated for the reconstruction of large fistulas and excessive palatal scars after several unsatisfactory procedures. The flap offers several advantages since it presents abundant tissue, excellent irrigation and easy rotation. The result was satisfactory.

ABSTRACT

Pleomorphic adenoma is the most common benign neoplasm of salivary glands. Their common location is in parotid gland, however, a lower percentage of these tumors might occur in minor glands. The epidemiology of this tumor shows that adults are the most affected, with rare occurrence in children or adolescents. We present the case report of pleomorphic adenoma located on the palate of a 10 year old. Excisional biopsy of the lesion followed by histopathologic examination of the biopsy specimen revealed ductal structures surrounded by plasmacytoid mioepithelial cells within a myxoid stroma, the final diagnosis corresponded to Pleomorphic Adenoma. Early detection and excision of this lesion in children are important to minimize potential recurrences or malignant transformation.

ABSTRACT

Pleomorphic adenoma is a benign mixed tumor, which is composed of myoepithelial and epithelial cells. A fibrous capsule separates these cells from the surrounding tissues. Pleomorphic adenoma has unusual histopathologic features. It is the most common benign tumor affecting both major and minor salivary glands. Parotid salivary gland is affected mostly in the major group, and palate is the most common site affected in minor salivary glands. The upper lip is the second most common site followed by buccal mucosa. Less than 3% of the salivary gland tumors account for head and neck tumors. In few cases, benign pleomorphic adenoma may turn malignant also. In this case report, a female patient aged 32 years who reported with a complaint of painless swelling in the palate is presented.

ABSTRACT

Pleomorphic adenoma (PA) is the most common neoplasm encountered in major and minor salivary glands. Intraorally, it is most frequently developed in the palatal glands. Histologically, it is characterized by a diverse architecture comprised of epithelial stromal elements mixed with mucoid, myxoid, or chondroid fibrohyaline. A PA does not generally present gender bias and can occur at any age with the same clinical behavior. It is usually a round, slow-growing, painless tumor, which is firm upon palpation. We reported two cases of adult patients who were treated using transoral resection at San Juan de Dios Hospital in La Serena.

ABSTRACT

(AU)

Oral squamous cell carcinoma (CEO ) has a marked tendency to invade the bone when located in the palate and lip . The precise mechanism of this invasion is still not completely described , although the literature suggests that the bone destruction ABSTRAT , invasion and metastasis is mediated by osteoclasts and not directly by carcinoma cells . The molecules RANK / RANKL / OPG are fundamental in oesteoclastogênese, and IL-6 that regulates its expression. The objective of this research was to evaluate the immunohistochemical expression of factors of bone resorption ( RANKL and OPG ) and cytokine ( IL - 6 ) in squamous cell carcinoma of the hard palate (with bone invasion ) and tongue (without bone invasion ) , correlating with the clinicopathological parameters and prognosis. The sample consisted of 30 squamous cell carcinoma with bone invasion (located on the palate) and 31 without bone invasion (located on the tongue). Were evaluated and the average intensity of the tumor cells , stromal and inflammatory immunostained for anti-RANKL antibodies , anti- OPG and anti -IL- 6 at the invasive front and center of the tumor. The score (s) of the immunoreactivity of cells was established by multiplying the percentage of positive cells (P) for marking intensity value (I) (S = P x I) in five fields (× 400). The analysis of the expression of RANKL protein was expressed significantly higher (p = 0.002) in inflammatory cells, there is a trend to higher expression in squamous cell carcinomas of the hard palate. Among the clinicopathologic parameters was observed association of RANKL with the worst prognosis, with statistical significance only for the advanced stage of the tumor (p = 0.033). The OPG showed weak expression both in cases of tongue (0.77 ± 1.85) and hard palate (1.32 ± 2.48), with no statistical significance (p > .05). In relation to clinicopathological parameters OPG tended association with worse prognosis, with a statistically significant association for death (p = 0.048) and perivascular invasion (p = 0.047). IL- 6 was expressed significantly more (p < 0.001) in tumor and inflammatory cells in squamous cell carcinoma of the hard palate. In addition, among the clinicopathologic parameters, IL- 6 showed a tendency of association with good prognosis, with a statistical difference for the absence of metastasis and tumor cells (p = 0.020), stromal (p = 0.027) and inflammatory (p = 0.017). Based on the results it can be concluded that IL -6 may be used as a marker of oral squamous cell carcinoma with bone invasion, and the ratio RANKL / OPG is changed in oral squamous cell carcinoma. ABSTRACT

Introduction: Palatal torus, or torus palatinus, is a benign bone alteration that may cause some discomfort during phonation or swallowing. When its growth or persistent exposition produces unpleasant symptoms, it must be surgically removed. Case presentation: We treated an 82-year-old female patient who consulted for a painful ulcerous lesion she had had for a year and which produced halitosis and discomfort when swallowing. During the oral cavity physical exam we observed a hard protuberance on the midline. It showed a perforation and ulceration of the lateral posterior palatal mucosa on the left side. We considered the following diagnosis: palatal torus, osteoma, pyogenic granuloma, or a soft-tissue neoplasia. We decided to surgically remove it and to perform a histopathological examination. Clinical evolution was satisfactory with complete resolution one month after surgery. The histopathological examination showed hyperostosis along with chronic inflammation of the soft tissues and a simple hyperplasia of the mucosa, which in turn indicated a palatal torus. Discussion: We report an unusual case of spontaneous exposition of a palatal torus which took up almost all of the hard palate area. Its resection is described and we inform the histopathological findings. In the literature review, we did not find a previous report of a spontaneous exposition of a palatal torus.

ABSTRACT

PURPOSE: To compare the hard palate dimensions of nasal-breathing children, mouth breathers from obstructive etiology, and habitual mouth breathers. METHODS: The sample comprised 76 children, 37 boys and 39 girls, with mean age of 9.32±1.16 years, distributed according to the diagnosis of breathing mode and to the etiology of mouth breathing. Plaster cast models of the subjects' superior dental arch were obtained in order to measure the hard palate with a digital caliper. Measurements of transverse, vertical and anteroposterior palatal length were taken. The hard palate measures were compared among the groups through statistical analysis. RESULTS: The comparison of hard palate dimensions observed in nasal and mouth breathers showed differences regarding the distance and depth of second premolars, and the distance of first molars. Differences were also found between the groups of mouth breathers regarding the hard palate depth at the level of canines. CONCLUSION: Mouth breathers showed narrower hard palate at the level of second premolars and first molars, and deeper palate in the level of second premolars, when compared to nasal breathers. It is evidenced that habitual mouth breathers presented deeper hard palate at the level of canines, when compared to mouth breathers from obstructive etiology.

ABSTRACT

A 58-year-old man presented with itchy papular eruptions all over the body since 15 years. Intraoral examination revealed raised papular lesions on the labial mucosa, hard palate, and tongue. The histopathology of the oral and skin lesions was confirmative of Darier disease (DD). This patient also showed esophageal involvement, which was confirmed histopathologically. Such a presentation of DD, with oral and esophageal involvement, is rare.

ABSTRACT

Introduction: Maffucci’s Syndrome is a rare nonhereditary mesodermal dysplasia consisting of multiple haemangioma of the soft tissue and enchondromas, mostly affecting phalanges and long bones. The syndrome can also be associated with a variety of other benign and malignant tumors. Case report: Here we report a case of Maffucci’s Syndrome and haemangioma of lip and palate which is rare in this syndrome. This case report describes an 18 year old boy with multiple nodular soft tissue swellings involving the anterior hard palate and lower labial mucosa and bony abnormalities (enchondromas) involving the lower limb.

ABSTRACT

ABSTRACT

Background: Millions of people worldwide consume carbonated drinks every day. The effects of these drinks on hard tissues in the mouth have been proved beyond doubt. Only a little has been done so far to assess the effects of carbonated drinks on oral soft tissues. This study was an attempt to assess the effect of carbonated drinks on oral wound healing. Materials and Methods: Twenty female Wistar rats were considered for the study. A circular wound was created on the palate and the animals were divided into two groups (experimental and control group). The experimental group animals were fed with a commercially available carbonated drink instead of water, and two animals from each group were euthanized at 3, 7, 14 and 21 days. Wound site was assessed morphometrically and histologically. Results: There was a marked difference in the healing pattern between the experimental group and control group animals. Control group animals showed a normal healing pattern with formation of a fibrous connective tissue at the end of 21 days. In the experimental group, healing was delayed and disrupted. The wound site showed a definite palatal perforation in experimental group animals after 14 days, but osteoclasts were not noticed in the histological sections. Conclusion: Consumption of carbonated drinks can disrupt oral wound healing. Results suggest that the bone changes seen in experimental group samples are not mediated by osteoclasts, and acidity of the carbonated drinks could be one of the reasons for these changes.

ABSTRACT

OBJECTIVE: To present a case and discuss the clinicopathological characteristics of nasopalatine duct cyst and discuss the etiology, diagnosis, treatment and prognosis, with a review of the literature. DISCUSSION: Nasopalatine duct cyst occurs in approximately 1% of the population. Presentation may be asymptomatic or include swelling, pain, and drainage from the hard palate. Surgical treatment was carried out under local anesthesia and comprised the dissection and removal of the cyst adopting a usually palatine approach.
